Your Initial Visit
Our team is committed to providing the very best neurosurgical care. During your first visit we would like you to know a few things before coming in order to serve you in the best possible way.
Depending on your insurance plan, you may need a referral from your Primary Care Physician (PCP) to be seen by a specialist.
Please arrive 20 minutes before your in-office visit to fill out all necessary forms. If you would like to fill out forms ahead of time, please download the form linked below and bring it along with you to your visit.
Things you should bring:
- A government issued photo ID
- Insurance card
- Any recent of past imaging if you have it including MRI, CT, X-rays.
- A list of prior treatments
